O projektu

Cílem projektu je pomoc dětským nemocným rozvojem moderních šetrných terapeutických programů, kvalitních volnočasových aktivit i zlepšení jejich pobytových služeb a osobní volnosti. Jedná se o projekt s dlouhodobými účinky pro duševní zdraví dětí a následně i celé společnosti, z rozpočtu Ministerstva zdravotnictví lze hradit jen základní zařízení, příspěvky zdravotních pojišťoven jsou malé, navíc často dochází ke zpoždění plateb, léčebna tedy bohužel sama nemá dostatečné finanční prostředky pro realizaci tohoto projektu. Lze ho realizovat jen na základě podpory z těchto fondů.

Zde používané léčebné postupy jsou celkově v souladu s právními předpisy a texty týkajícími se tohoto oboru a sledují moderní vývoj v psychiatrii. Tato finanční investice by zkvalitnila a zmodernizovala již používanou terapii, jako např. arteterapii, muzikoterapii a teatroterapii, které by se díky investicím do materiálu staly přitažlivější a zajímavější a zavedla některé nové terapie, které se zejména z finančních důvodů nemohly uplatnit: arterapie aplikovaná na keramiku, aminoterapie se rozšíří, neboť se plánuje lepší zabezpečení přístupu ke zvířatům, výrazně se zlepší sportovní vyžití a jeho blahodárné účinky na celý organismus a zavede se rehabilitace a relaxace. Tato finanční investice by výrazně zpříjemnila pobyt nejmladších pacientů v léčebně, a příp. ho zkrátila.

Celkově si psychiatričtí pacienti žádají velmi šetrný a ohleduplný přístup, u malých pacientů zvláště je nutné volit k nemocnému přívětivou léčbu, doplněnou různými moderními terapiemi tak, aby se malý pacient co nejvíce odpoutal od skutečnosti, že je v léčebně. Velkou úlohu zde hraje i způsob trávení volného času. Při hře se děti uvolní, zlepší komunikaci s okolím, naučí se lépe poznávat okolní svět a vyrovnávat se sním. V rámci projektu vznikne celková infrastruktura pro šetrné terapeutické postupy. Zlepší se vybavení herních místnosti, zřídí se knihovna, plánuje se stavba lehké lezecké stěny, keramické dílny, vybavení pro muzikoterapii, arteterapii a muzikoterapii, počítá se se zakoupení stolního tenisu, velkých šachů, atd.

Díky různorodým aktivitám se pobyt v léčebně pro mladé pacienty zpříjemní. Při hře se dětští pacienti uvolní, zlepší se následně i působení léčebných metod a celková doba léčby se zkrátí. Kladné návyky spojené s trávením volného času – naučí se novou hru, získají v něčem zálibu – si mladí mohou přenést i do civilního života.

Harmonogram projektu

Nejprve se plánuje úprava vnitřních prostor, zakoupení vybavení pro společenské hry, hraček pro nejmenší pacienty a velkých plyšových zvířat pro kontaktní terapiii. Hned od spuštění projektu tak malí pacienti pocítí pozitivní změny.V první fází lze též vytvořit podmínky pro muzikoterapii, děti budou moci hrát na jednoduché hudební nástroje, plánuje se zakoupení Orfových nástrojů. Vznikne dobře vybavená herní místnost, knihovna s možností půjčování knih časopisů a denního tisku. Bude znásoben počet počítačů tak, aby děti byly v lepším spojení s rodiči, i se školou, mohly studovat cizí jazyky pomocí internetu. Plánuje se i nové zařízení kuchyňky a zakoupení tkalcovského stroje.

Ve druhé fázi se počítá s venkovními pracemi, které jsou ovšem závislé na ročním období. Tato etapa se týká zejména výstavby zookoutku, zřízení skleníku. V plánu je lezecká stěna.
V další fázi se počítá s pořízením malé bezpečné keramické pece a hrnčířského kruhu pro samostatnou keramickou tvořivost mládeže. Jedna místnost s pódiem a kulisami by měla být věnována theatroterapii. Postupně se začne i s úpravou vnějších prostor. Poslední fází realizace projektu bude instalace bezpečnostního systému.

Popis projektu a jeho aktivit

Projekt zahrnuje tři klíčové aktivity:

  • modernizace a doplnění léčebného procesu;
  • kvalitní, zajímavé a duševnímu zdraví prospěšné volnočasové aktivity;
  • zlepšení svobody a umožnění osobní svobody a umožnění větší volnosti pohybu dětských klientů zavedením bezpečnostního systému a dále
  • publicitu projektu.

Modernizace a doplnění léčebného procesu
Lůžková péče o děti a mládež s psychiatrickými onemocněními je specifickou a velmi náročnou oblastí psychiatrie, která si žádá ohleduplné a moderní terapeutické postupy a programy, na základě kterých se léčebný proces stane nejpřívětivější a „domáčtější“. Pro jejich využití je ovšem třeba upravit vnitřní prostory a vybudovat nová zařízení. Vytvoří se tak podmínky pro provozování, muzikoterapie, arteterapie, teatroterapie, plánuje se zakoupení malá keramické pece, vybudování a zařízení kuchyňky, zakoupení velkých plyšových zvířat pro kontaktní terapii. Tyto jednotlivé metody, budou synergicky působit v rámci celého léčebného procesu. Vznikne tak infrastruktura pro šetrné terapeutické postupy.

Kvalitní, zajímavé a duševnímu zdraví prospěšné volnočasové aktivity
Nejčastější diagnózou u malých nemocných jsou poruchy chování. Vzhledem k tomu, že průměrná dobu pobytu jednoho dětského klienta je dosti dlouhá, má kvalitní a příjemné prožití volného času mimořádný význam. Nejen se malým pacientům zpříjemní pobyt v léčebně, ale v rámci těchto aktivit se uvolní, snáze přijmou léčebné postupy a přispěje se k jejich lepší resocializaci po návratu domů. Aktivity našeho projektu pojímají problematiku trávení volného času komplexně. Jednotlivé činnosti a postupy v této aktivitě se navzájem propojují a doplňují, jejich působení je ve svém důsledku velmi komplexní. Pozitivní návyky spojené s trávením volného času si děti ponesou i po návratu z léčebny.

Zlepšení svobody a umožnění osobní svobody
Třetí a neméně významnou částí projektu je zavedení bezpečnostního systému, díky němuž se dětští pacienti budou moci volně pohybovat v prostoru léčebny a výše uvedené terapie a volnočasové aktivity budou moci působit komplexněji a vůbec etapy projektu tak budou mít pozitivní dopad na celkový stav duševního onemocnění. Jedná se o přístupový systém pomocí identifikačních karet s možností přidělení různých oprávnění k přístupu a monitorovací systém pomocí kamerového systému a zvukových čidel. Kamerový systém by měl být umístěn výhradně ve veřejně přístupných prostorách (chodby, herny jídelny, venkovní hřiště) a má sloužit k možnosti rychlé reakce personálu v případě nutnosti (úraz, zdravotní potíže apod.). Diskrétní zóny (ložnice, učebny,koupelny, WC, pracovny lékařů, psychologů, sester apod.) nebudou samozřejmě monitorovány technikou vůbec.

Publicita projektu
Publicita Základními cíly publicity bude poukázat na samotnou existenci Finančního mechanismu EHP/Norsko a zvýšit o něm povědomí a propagace samotného projektu i toho, z jakých finančních zdrojů je projekt financován. Publicita bude zajišťována dodavatelsky. Cílovými skupinami bude odborná veřejnost, částečně laická veřejnost, dále zaměstnanci léčebny a naši klienti se svými rodiči a osobami jim blízkými. V neposlední řadě i ostatní zdravotnická zařízení v České republice s příslušným zaměřením.

Psychiatrická léčebna Bohnice (Psychiatric Hospital Bohnice)

Psychiatric Hospital Bohnice was founded in 1905; it is a state allowance organization funded by health insurers according to the number of treatment days of the patients insured. Ministry of Health of the Czech Republic is the founder of Psychiatric Hospital Bohnice and it provides subsidies to it for structures, machines and fixed-asset-like equipment. Psychiatric Hospital Bohnice is the largest in-patient mental institution in the Czech Republic. It has currently 1421 beds including the beds in the Long-Term In-Patient Care Unit.

The in-patient section of the of the children’s ward of the Psychiatric Hospital Bohnice has currently 44 beds for child clients. It focuses on in-patient as well as out-patient treatment of clients with mental diseases. Their ages usually ranges between 8 and 15 years. The most frequent diagnosis in our young patients is behavioral disorders, ADHD (Attention – Deficit Hyperactivity Disorder), and psychoses. The children’s ward has an average duration of stay of one patient 46 days. The average occupancy of beds is 88 %.

Initial Situation and Justification of the Implementation of the Project

The goal of the project is to help the child patients by development of modern considerate treatment programs, quality leisure activities and by improvement of their in-patient services and personal freedom. It is a project with long-term effects on the mental health of children and subsequently the entire society; the budget of the Ministry of Health can only cover the basic equipment, the contributions from the health insurers are small, and often the payments are delayed, and unfortunately the hospital itself does not have sufficient funds to implement this project. It can only be implemented based on the aid from these funds.

The treatment procedures used here are generally in accordance with the legal regulations and texts pertaining to this field and follow modern trends in psychiatry. This financial investment would improve and modernize the therapy that is already used, e.g., art therapy, music therapy and theater therapy, which would become more attractive and interesting thanks to the investments in the material, and it would result in introduction of some new therapies, which could not be used mainly for financial reasons: art therapy using ceramic, animal-assisted therapy will be extended because enhances access to animals is planned, sports activities and its beneficial effects on the entire organism will be improved significantly, and rehabilitation and relaxation will be introduced. This financial investment would make the stay of the youngest patients in the hospital significantly more pleasant, or it would reduce it.

In general, psychiatric patients require very sensitive and considerate approach; in child patients it is necessary to choose a patient-friendly treatment that includes modern therapies, so that the child patient could free himself or herself as much as possible from the fact that he or she is in a hospital. The way the patients spend their free time plays a very significant role. During play children get relaxed, improve communication with those around them, learn to get to know the surrounding world better and to cope with it. An entire infrastructure for considerate therapeutic procedures will be created within the project. The equipment of the play rooms will improve, a library will be established, an easy climbing wall is planned, ceramic workshops, equipment for music therapy, art therapy and music therapy is planned, acquisition of a table tennis set, large chess sets are also planned, etc.

Thanks to the various activities, the stay in the hospital will be more pleasant for the young patients. The child patients get relaxed during play, and the effects of the treatment methods subsequently also improve, and the total time needed for treatment will be reduced. The young patients can take the positive habits associated with the spending of the free time – learning a new game, acquiring a new hobby – with them to the civil life.

Analysis of Necessity of the Project

The necessity of the project activities is urgent and of a long-term nature – this is confirmed by our everyday experience as well as the experience of others and generally accepted documents of the European Union and the Czech Republic.

These needs are mentioned in the Green Book – Improvement of mental health of the population – on the way to the strategy of mental health for the European Union, which deals with public health based on article 152 of the EC Treaty, in particular in chapter 3., page 5, chapter 4., page 5 and chapter 6.1.1, page 8.
Also Declaration of Mental Health for Europe in chapter 8., paragraph 4. as well as the Action plan of mental health of the population, chapter 4., page 5.
Also in the Czech Republic the need is implied by Health 21, particularly chapter 3.3., goals 4. And chapter 4.2, goals 6.

Resolution of the EU Council (2000/C 86/01) recommendation of the Council of Europe acknowledge the significant role of supporting mental health and the unfortunate link between mental health disorders and social marginalization, unemployment, homelessness and disorders related to the abuse of alcohol. All these problems can be a result of neglecting some symptoms in the childhood. Psychiatric care must therefore be comprehensive, as effective as possible, it must provide a broad range of therapies, in particular in the case of child patients. If the young generation is happy, mentally relaxed it can contribute to good development of the entire society in the future. If not, a lot of money will be spent on the consequences related to crime, drug abuse, violence, etc.

For young patients who most often suffer from behavioral disorders, ADHD and psychosis it is very important that they feel as good as possible in the hospital, that they could pursue the same leisure activities like at home, that their movement is restricted as little as possible. Their problems can then be more easily unblocked, and they will be open to accept new stimuli, and the treatment will have better and faster effects on them.

Project Implementation Schedule

Modification of the interior areas, purchase of equipment for games, toys for the youngest patients and large plush toy animals for contact therapy are planned first. The young patients will therefore see the positive changes immediately after the launch of the project. During the first phase it is also possible to create the conditions for music therapy, the children will be able to play simple musical instruments, purchase of Orf instruments is planned. A well-equipped playroom, library where books and magazines and daily newspapers can be checked out will be established. The number of computers will be multiplied, so that the children can have better contact with parents and the school, so that they can study foreign languages using the Internet. New equipment for the small kitchen and a loom are planned.

During the second phase we envisage outdoor works, which, however, are dependent on the year’s season. This stage of the project pertains primarily to the construction of the small zoo establishment, construction of a greenhouse. A climbing wall is planned as well.

In the next phase we envisage purchasing a small safe ceramic kiln and potter’s wheel for independent ceramic creativity of the youth. One room with a stage and sets should be dedicated to theater-assisted therapy. The outdoor areas will be gradually renovated as well. The last phase of the implementation of the project will be installation of a security system.

Technical and Technological Solution of the Project

The technical and technological solutions of the project will respect all the legislative regulations and laws of the Czech Republic as well as all legislative regulations of the European Union.
The technical solutions of the greenhouse, arbor, climbing wall and the modification of the zoo establishment for animal-assisted therapy are provided in separate attachments (P 18 Specifications for the greenhouse, P 19 Specifications for the security system, P 20 Specifications for the climbing wall and P 21 Specifications for the zoo corner and animal-assisted therapy – documents are available at NFP)

Evaluation of the Effectiveness and Sustainability of the Project

Evaluation of the effectiveness of the project is based on the position of the European economic and social committee on the topic Green Book – Improvement of mental health of the population, fulfillment of the needs of children and the youth is a priority.

The care of the mental health can only be effective when it presents a system of complete and effective care.
For healthy development of children and the youth it is necessary to ensure effective use of their free time. Based on this project, a complete program of free-time activities covering children of various age categories will be created.

Such infrastructure as well as the system of complementing the care by improvement of the mental health will be of long-term nature.
The project outputs will be sustainable in a long term because both the costs of maintenance as well as the cost of the energy or consumables will be borne by the applicant from its own budget.

Financial Cash Flow Analysis

According to the cash flow statement attached, we can monitor balanced use of funds. The EHP grant, which constitutes 85% of the funding resources, will be used first. In the final stage we envisage funding from our own resources (the remaining 15 %). In the last continuous request for payment we will take into account the deposit, and the above-mentioned own funds will be used for funding.

Risk:
Unskilled team, lack of skilled employees.

Solution:
The project will involve specialists in the field of psychiatry as well as rehabilitation/recovery and care of ill children. Mental Hospital Bohnice is the largest in-patient mental facility in the country and has enough highly skilled employees.

Project with the wrong topic.

Solution:
The project has very needed topic and is compliant with large volume of documents and resolutions.

Risk:
Lack of interest within the target group.

Solution:
The project has been designed based on long-term experience with working with the target group; inquiries as well as practical experience confirm enormous interest in the project activities.

Risk:
Negative impact on the environment.

Solution:
The project authors envisage that the principles of sustainable development will be strictly followed throughout the project. Minimum texts will be on paper, waste will be sorted, the appliances and equipment with lower power consumption will be chosen, and recycling will be consistently promoted.

Project Evaluation Based on the Results of the Study

Based on the results of the study it is possible to say that the project concerns a very needed topic of great social importance and significant impact.
It is needed for the target group of child psychiatric patients, its individual activities are anticipated and even demanded by the target group. It also complies with many documents and recommendations, both global and European ones and national ones.
The project is put together thoughtfully, well and with detailed knowledge of the environment and the target group. The project activities and their individual steps and procedures are logically connected, related, they complement each other and form a single organic unit.
The schedule of the implementation of the project is set carefully, while respecting the local conditions.
The project management will include experienced people; the management has clearly defined roles and responsibilities.
According to the study, the project will be effective, respecting the principles of good management, and its positive outputs will be sustainable on long-term basis.
The financial aspect of the project is well secured, in accordance with the schedule of use of the funds and the financial cash flow analysis and the economic cash flow analysis it shows that the project is realistically financially secured and will not suffer from a lack of funds during its course.
The risks are low and the risk-eliminating measures are well developed.
